Restaurateur and columnist Bruce Buschel has recently compiled a fantastic list of 100 Things Restaurant Staff Should Never Do.
Kosher restaurants, even the expensive ones, could especially do with a much more intense training of their staff on how to interact with customers. For most people, going out to eat to a high-end restaurant is a treat saved for special occasions like birthdays and anniversaries. You want, and expect, to be treated like a queen. You are paying for it after all.
